APPOINTMENTS
All new clients are scheduled for an initial diagnostic interview. Your initial appointment may
take anywhere from an hour and a half to two hours, depending on how much information needs to be discussed, so please plan
accordingly.
Prior to
your first appointment you will need to sign the appropriate authorization forms to proceed with the services that you are
requesting. Please plan on arriving to your appointment at least fifteen minutes prior to your scheduled
time to complete these forms. You may also download the forms here and bring them with you on the date of your appointment.
During the initial interview you will
be asked to share details about your current concerns as well as historical information, such as health history, family history,
educational history, social history, and previous mental health services received. Please plan on bringing
copies of whatever documents or previous records you would like for Dr. Gómez to review and take into consideration.
Because your initial appointment will be spent focusing on obtaining the aforemtnioned information, Dr. Gómez
may request the opportunity to keep your documents for a thorough review after you have left the office. Therefore,
please plan on bringing in photocopies that you would be comfortable leaving behind.
At the end of the initial interview, Dr. Gómez will offer
specific recommendations about what type of services or testing may be indicated for your particular circumstances.
On rare occasions, if prior arrangements are made, your initial interview may be followed by testing on that same date.
However, generally, a followup appointment will be scheduled on a later date to initiate psychological testing or psychotherapy.

The office is generally open between 8:30a.m. and 4:30p.m.
Monday-Friday.
We are not open on evenings,
weekends, or holidays.
INSURANCE
Dr. Gómez is an authorized provided on most of the major
insurance plans. As a courtesy, claims will be filed on your behalf for those companies that she is contracted
with. You will be responsible for your portion of the payment (i.e. deductible, co-pay, co-inbsurance)
on the date that services are rendered. For further information, please verify with your particular insurance
company what your current mental health benefits are and that Dr. Gómez is an authorized provider on your plan.
Dr. Gómez accepts cash and
checks only. All payments are due at the time that services are rendered.
